Investigations in genital warts serve two purposes: excluding the dangerous mimics and assessing the broader oncogenic risk that HPV carries. Because condylomata lata can masquerade as warts, VDRL/RPR to exclude secondary syphilis is appropriate whenever the morphology is atypical or flat-and-moist. As with every STI presentation, offer HIV testing. For women with genital warts, arrange cervical screening (Pap smear) referral, since exposure to genital HPV — including possible concurrent high-risk types — raises the relevance of screening for cervical dysplasia. HPV genotyping can identify high-risk types but is not required for routine clinical management of visible warts. A biopsy is reserved for suspicious lesions, and an anal Pap may be considered for perianal warts in high-risk individuals. The point of this step is to look past the warts themselves to the person's overall oncogenic-HPV risk and to act on it through screening and vaccination counselling.

Management of genital warts combines an individualised choice of treatment with the education that gives the consultation its lasting value, and a key safety rule governs the choice. Treatments fall into patient-applied and provider-applied groups. Patient-applied options include podophyllotoxin (0.5% solution, applied in cycles) — effective for accessible warts but contraindicated in pregnancy because it is teratogenic — and imiquimod (5% cream, an immune-response modifier applied over several weeks, also avoided in pregnancy). Provider-applied options include trichloroacetic acid (TCA) and cryotherapy with liquid nitrogen, both of which are safe in pregnancy and suitable for many sites, and surgical excision or electrocautery for large or resistant warts. Second, before you ever hand a patient podophyllotoxin or imiquimod, ask about pregnancy, because podophyllotoxin is teratogenic and the pregnancy-safe choice is a provider-applied agent such as TCA or cryotherapy. And never let a 'minor' wart consultation pass without offering HPV vaccination counselling and cervical screening — the same virus family causes cervical cancer.
