INSTRUCTIONS
Antenatal care (ANC) is the organised health-care delivered to a pregnant woman from conception to the onset of labour. Systematic ANC is India's single most powerful lever for reducing the 103 maternal deaths per 100,000 live births (SRS 2018–20) and the 28 neonatal deaths per 1,000 live births that persist despite significant progress. This module builds the cognitive and procedural scaffold you need to conduct structured ANC visits, identify pregnancies that are deviating from the normal trajectory, and match the level of care to the level of risk — the essence of the inverted pyramid of care.

CLINICAL SCENARIO
Priya, a 22-year-old primigravida from a rural district in Rajasthan, presents to your peripheral health centre at 32 weeks of gestation. She has had no formal antenatal visit since her first trimester urine pregnancy test confirmed the pregnancy. Antenatal care is the domain where preventive obstetrics and clinical medicine converge. As a final-year medical student and future clinician, you will conduct ANC in community postings, primary health centres, and district hospitals. The ability to enumerate ANC objectives, apply the visit schedule, screen for high-risk factors, and deploy the inverted pyramid framework is directly assessed in MBBS Professional examinations under competency OG8.1 (KH level — knowledge and how). More immediately, a well-conducted ANC contact can detect pre-eclampsia, severe anaemia, malpresentation, diabetes and IUGR weeks before they become emergencies — making this a life-saving clinical skill, not an administrative task.

Why Antenatal Care Matters: The Clinical Case for Structured Surveillance
Antenatal care is a package of health-care interventions provided to a pregnant woman during gestation, with the dual purpose of promoting maternal and fetal wellbeing and detecting complications early enough to prevent morbidity and mortality. The case for structured ANC is built on epidemiological reality: in India, haemorrhage, hypertensive disorders and sepsis account for over 60% of maternal deaths, and the majority of these deaths are preventable when the underlying conditions are detected and managed before the acute event. The global Sustainable Development Goal (SDG) target 3.1 calls for a maternal mortality ratio (MMR) ≤70 per 100,000 live births by 2030; India's current MMR of approximately 103 (SRS 2018–20) means that ANC quality is a direct contributor to whether this target is met.

- To screen for and manage complications — systematic recording of blood pressure, weight gain, urine protein, haemoglobin, blood group and indirect Coombs test, blood sugar (DIPSI/IADPSG), and STI screening (VDRL, HIV) allows early identification and treatment of pre-eclampsia, anaemia, gestational diabetes, isoimmunisation and congenital infection.
- To promote maternal nutrition and prevent deficiency states — iron-deficiency anaemia (Hb <11 g/dL) is present in ~45% of pregnant Indian women at delivery; daily IFA supplementation (60 mg elemental iron + 500 µg folic acid) and calcium supplementation (1.5–2 g/day for pre-eclampsia prevention in low-intake populations) are among the highest-yield preventive interventions globally.
- To provide immunoprotection — tetanus immunisation via Td schedule protects both mother and neonate from tetanus neonatorum, which remains a cause of preventable neonatal death in areas with low vaccination coverage.
- To prepare for childbirth and emergencies — birth preparedness counselling (identifying a skilled birth attendant, transport plan, blood donor, savings), danger sign education, and registration of the pregnancy in the health system all reduce the 'three delays' (decision, reaching facility, receiving care) that contribute to preventable maternal death.

Governing Principles: Physiology of Pregnancy and the Inverted Pyramid

This section articulates the governing principles that make each ANC assessment meaningful rather than ritualistic.
The haemodynamic changes of normal pregnancy — a 40–50% rise in plasma volume that exceeds the 20–30% rise in red cell mass, creating the physiological anaemia of pregnancy — mean that a haemoglobin of 10–11 g/dL in the second trimester may be normal, whereas the same value in a non-pregnant woman would be mildly anaemic. Critical supplementation and immunisation details that are examination essentials:
- IFA: 60 mg elemental iron + 500 µg folic acid per tablet, one tablet daily throughout pregnancy and for 100 days postpartum; administered under the national IFA supplementation programme.
- Calcium: 1.5–2 g elemental calcium per day (typically 500 mg three times daily) from 20 weeks onward for women with low dietary calcium intake, as per WHO 2016 recommendation for pre-eclampsia prevention.
