CLINICAL SCENARIO

You will write a structured clinical essay integrating the physiology of labour onset, the mechanism of labour, monitoring using the partograph, conduct of normal vaginal delivery, active management of the third stage, and the principles of respectful maternity care. This assignment prepares you for your intrapartum log-book and for explaining labour management to patients.

Instructions

Write a structured essay covering the following: (1) the physiological triggers of labour onset; (2) the seven cardinal movements of labour in occipito-anterior presentation and their clinical correlates; (3) how the WHO partograph is used to monitor labour progress and identify deviations; (4) the key steps in conducting a normal vaginal delivery; (5) active management of the third stage; and (6) how respectful maternity care principles are applied at each stage. Use clinical examples drawn from your ward experience where possible. Ground your answer in standard references (DC Dutta's Textbook of Obstetrics or Shaw's Textbook of Gynaecology and Obstetrics). Cite at least two references.

What to Submit

Section 1 — Physiology of Labour Onset (approx. 150 words)

Guidance: Explain the hormonal and biochemical cascade that initiates normal labour at term: the role of the fetal HPA axis, oestrogen-progesterone balance, prostaglandins (PGE2 for cervical ripening, PGF2α for myometrial contractions), and oxytocin. Mention the three stages of labour and their time limits for primigravida and multigravida.

Section 2 — Mechanism of Labour in OA Presentation (approx. 200 words)

Guidance: Describe all seven cardinal movements in sequence: engagement, descent, flexion, internal rotation, extension, restitution, and external rotation. For each, explain why it occurs in terms of the pelvic plane at that level. Mention the clinical examination finding that corresponds to each movement (sagittal suture, station, head position on VE).

Section 3 — Monitoring with the WHO Partograph (approx. 150 words)

Guidance: Explain the three sections of the WHO partograph (fetal condition, labour progress, maternal condition). Describe the alert line and action line — their meaning and the required response when each is crossed. Give one example of an abnormal partograph finding and what it should prompt.

Section 4 — Conduct of Normal Vaginal Delivery (approx. 200 words)

Guidance: Describe the step-by-step conduct of a normal vaginal delivery: preparation, second-stage monitoring, delivery of the head (controlled, Ritgen manoeuvre), checking for cord around the neck, delivery of shoulders (anterior then posterior), clamping and cutting the cord, and initial neonatal resuscitation/assessment.

Section 5 — Active Management of the Third Stage (approx. 150 words)

Guidance: Describe the three components of AMTSL: uterotonic (oxytocin 10 IU IM within 1 minute of delivery), controlled cord traction with uterine counter-pressure (Brandt-Andrews), and uterine massage. State the signs of placental separation. Mention ergometrine contraindication in hypertension.

Section 6 — Respectful Maternity Care in Labour (approx. 150 words)

Guidance: Apply the White Ribbon Alliance RMC Charter principles to the intrapartum period. Give concrete examples of respectful and disrespectful behaviours during labour. Mention the right to a birth companion, the right to information and consent before every examination, the right to privacy, and LaQshya/MuSQan as India's quality-improvement frameworks.
